A sub center is the most peripheral and first point of contact between the primary health care system and the community.
Sub-centres are entrusted with interpersonal communication related functions to provide services in relation to behavior change and maternal and child health, family welfare, nutrition, immunization, diarrhea control and control of communicable diseases programmes.
Each phc can have 3 to 4 sub centers to 15 sub centers. The population of each sub center may be approximately 4500 to 5500. Each phc area can have phc area from 12 to 50 villages. Wadi workers and helpers do all the health related work in coordination with the main worker.All the staff of the sub center themselves do the health related work of all the people of their sub center population.
Complete monitoring of health oriented performance of all sub center villages of all phc in their area. And requirements are taken care of by phc.
