1. What is a free medical plan? ?
Free Medical Aid Scheme of Gujarat Government is a boon for poor middle class people suffering from critical diseases. This is one such plan. In which serious diseases like T.B.Cancer. Leprosy. Hiv (Aids). . Malnourished children. Anemic mother. And poor and middle class patients suffering from serious diseases like dengue are also given free of charge medicines for serious diseases by the government along with financial support for their nutrition until the medicines are started. It is called Free Medical Assistance Scheme.
2. Purposes of Free Medical Scheme.
The main objective of free medical scheme is to provide poor and middle class people living in rural and urban areas with TB cancer. Leprosy. Hiv (Aids). Malnourished children. Anemic mother. And if serious diseases like dengue apply, his family has to face a lot of problems. And in order to get out of such danger soon, the patient needs timely medicines along with nutritious diet. Thus, free medical assistance is provided by the government in all these diseases for the purpose that the patient can get nutritious food and recover quickly from serious illness.
3. Free medical aid is only available to whom. ?
All those patients of poor and middle 8th class of rural and urban areas who have started taking medicines for the following serious diseases. TB Cancer. Leprosy. Hiv (Aids) Malnourished children. Anemic mother. and dengue. All the above patients whose treatment (medicines) are started from government hospitals are eligible to get the benefit of free medical assistance scheme monthly or once.

(1) TB ...
In any government hospital, TB report positive patients will be given Rs . 500 is provided in the bank account of the patient.
All the following necessary documents are required to be submitted to your village health worker mphw / fhw to avail free TB medical assistance. who will fill their free medical assistance form and submit it to the applicable phc (primary health center) phc will submit the form to the taluka health office. Since then the patient gets the above 500 per month in his bank account.

(2) Cancer...
All patients undergoing treatment for any type of cancer are entitled to a free medical assistance scheme of Rs . Assistance of 1000 points is given in the patient's bank account.
All the following necessary documents are required to be submitted to your village health worker mphw / fhw to avail free cancer medical assistance. Who will fill their free medical assistance form and deposit it in the applicable phc (primary health center) phc will deposit all the forms in the taluka health office from then on the patient gets the above 1000 assistance every month in his bank account .

(3) Leprosy...
In any government hospital, if the report of leprosy is positive, all the patients who are put under treatment will be paid Rs . 800 per article for 11 months in the patient's bank account.
All the following necessary documents are required to be submitted to your village health worker mphw / fhw to seek free medical assistance for leprosy. who will fill up their free medical assistance form and submit it to the applicable phc (primary health centre). And the phc will deposit all the forms in the taluka health office. Since then, the patient receives an amount of 800 per month in his bank account.

(4) HIV (Aids) ...
In any government hospital, the HIV (Aids) report will be positive for all the patients who are put under treatment, Rs. per month till the treatment starts . 500 per month in the bank account of the patient.
To take free medical help of Hiv (Aids) all the following necessary documents visit the hiv counselor at every chc (community health center) ART center near your village and fill their free medical help form from there 500 per month in the patient's bank account, after which the amount will be deposited in the applicable district.
Other benefits available to HIV positive patients.
● BPL card is canceled if any person comes hiv positive. (Visit chc's hiv counselor for more information.)
● An hiv positive woman whose husband has died can get Antyodaya card. (Visit chc's hiv counselor for more information.)
● Aid kits are also distributed from time to time to hiv positive patients from Mamlatdar office. (Visit chc's hiv counselor for more information.)

150 under free medical assistance scheme to anemic expectant mothers (less than 7% hb) .
For the benefit of this scheme, the employee will fill the free medical assistance form with the health worker (mphw) and submit it to the phc (primary health center) and submit the form to the phc taluka health office. After that, the beneficiary gets the above assistance of Rs 150 in the bank account of the beneficiary.

{ 1 } Assistance to severely malnourished children in CMTC (Child Malnutrition Treatment Centre).
Under Dynamic Gujarat, Child Seva Center (CMTC) has started at all CHCs (Community Health Centers) of the state. In which severely malnourished children are admitted for 14 days and given nutritious food and treatment. The child is admitted in cmtc for 14 days. 1400 for 100 articles per day for 14 days and a total of 200 as travel fare and Rs. 1600 is given as assistance. Also after 14 days of cmtc treatment 3 times for follow-up check-up 300 for each check-up and the total amount is Rs. 900 is given. That is, 1600 for 14 days of treatment and 900 for total 3 examinations. Rs. 2500 (Two Thousand Five Hundred ) comes to help.

{ 2 } A pregnant mother at risk ...
A nutritious diet is absolutely necessary for all high-risk pregnant mothers. A mother's nutritious diet has a direct effect on her unborn child. Therefore, packets of Matrashakti are also given free of charge in the form of nutritious food from the Aganwadi center of all the villages to the expectant mothers.
Also, if the pregnant mother is at risk, then she can go to any government hospital under Pmsm (Pradhan Mantri Sarkar Matrutva Abhiyan) camp and get a total of 3 check-ups during pregnancy, if she goes to any government hospital by the state government for each check-up at the cost of transportation. Total of 100 articles. 300 assistance is credited to the beneficiary's bank account from the applicable Primary Health Centre. Contact your village health worker (fhw) nurse sister and Asha sister of your village to get benefit of this scheme after screening of high risk pregnant mother.

{ 3 } Dengue positive
Dengue positive reports are coming in any government hospital. Only for one time in the bank account of dengue positive patient Rs . 600 is credited to the patient's bank account.
All the following necessary documents are required to be given to the health worker mphw of your village to seek help for dengue positive. All those documents will be submitted by mphw to the applicable phc (primary health center). And the phc will deposit all the documents in the taluka health office. After that, a one-time assistance of 600 is deposited in the patient's bank account.

[1] Female sterilization. (T.L.)
Under Kutup Kalyan Yojana all rural and urban women undergo permanent female sterilization and the following assistance is given from the applicable phc (primary health center) to the beneficiary's bank account.
★ 1400 assistance is given to the beneficiary who has undergone female sterilization (tl) operation in a government hospital .
★ Assistance of 2200 is given to the beneficiary who has undergone female sterilization (tl) operation at any government hospital within 7 days of delivery .
★ Assistance of 1000 is given to the beneficiary who has undergone the operation by going with the health worker (mphw / fhw / asha) of his village and also in a recognized government recognized private hospital if he has undergone female sterilization (tl).

@ Death in hospital after sterilization (in case of family planning operation surgery) or within 7 days after discharge from operation hospital. 200000 two lakhs from the NHM budget and 200000 two lakhs from the state government budget and a total assistance of 400000 four lakhs is given.
@ In case of death due to sterilization within 8 days to 30 days after discharge from the hospital after sterilization operation, 50000 fifty thousand from NHM budget and 50000 fifty thousand from state government budget to give assistance of 100000 one lakh to the beneficiary . is coming .
@ In case of failure in female sterilization (sterilization) operation due to any reason, the beneficiary is given assistance of 30000 thirty thousand from NHM budget and 30000 thirty thousand from state government budget.

【 2 】Male Sterilization ..
Under the Kutup Kalyan Yojana, all male rural and urban males undergo male sterilization and the following assistance is provided from the applicable phc (primary health center) to the beneficiary's bank account.
★ Male sterilization in Govt Hospital on Koi. 2000 assistance is given to the beneficiary for the operation .

【 3 】 PPIUCD ( Akadi ) Copper - T
Under the family welfare programme, if copper-T (ppiucd) is placed within 48 hours of delivery as a non-permanent method of spacing between two children, the place where copper-T is placed should be phc (primary health center) or chc ( 300/- assistance of Rs. 300 /- from the Community Health Center) to the beneficiary is deposited into his/her bank account. The benefit of this assistance is to deposit the following documents where Copper-T is placed in phc or chc.

【 4 】 Antara ( Injection ) ......
Under family welfare program any woman non permanent method at any phc (primary health center) or chc (community health center) or sub center is given antara injection to keep distance between two children. This injection is taken every 3 to 4 months. So Rs 100 to the beneficiary at every Antara injection
The assistance is disbursed from the applicable Primary Health Center or Community Health Center into the beneficiary's bank account. The benefit of this assistance is to submit the following documents where Antara injection has been done in phc or chc.
